Appointments/Office Hours
Please call our office to schedule an appointment. If you are an established patient and need to be seen on a same day basis, please call our office as early as possible for a work-in appointment. We will do our best to accommodate you. Our office hours are Monday through Friday, 8:00am to 5:00pm, Saturdays as scheduled from 8:00am to 12noon. If you are unable to keep your appointment, please call us at least 24 hours in advance. Your cooperation in this matter allows us to see another patient who may require prompt attention. "No Shows" for surgical, laser and cosmetic procedures are charged 50% of the price of the scheduled procedure.
Emergencies
If you are uncertain about the urgency of your situation, please do not hesitate to call us. After office hours, our physician is available for urgent problems by calling our office number and following the prompts. The physician will be paged and your phone call will be returned as quickly as possible. Life threatening emergencies should be handled by calling 911 or visiting the nearest emergency room. Non-urgent calls will not be returned until the next regular business hours. Prescription refills are not considered an emergency and will not be filled after hours.
Prescription Refills
Please request prescription refills during regular office hours, and have the details of the prescription, medication name, strength and quantity, and the pharmacy phone number available. We will review and update your medical record before refills are authorized. Therefore, please allow 24 hours for prescriptions to be filled. Prescription refills are only completed during regular office hours. Please note that we are unable to prescribe medication that was not originally provided by our office or refill prescriptions older than 12 months.
Payment Policy
We are honored that you have chosen us for your medical care. Your well being is our first priority. As with any business, we must reconcile our financial relationship as well. You have access to a variety of health plans, and our financial relationship is dictated primarily by the health plan that have co-payments and deductibles, and it is our responsibility to collect these fees. This includes all co-pays, deductibles, and co-insurance percentages. We accept cash, checks, and Visa, MasterCard, Discover and American Express credit cards.
Please be advised that we file insurance claims to plans with whom we have mutual contract agreements. Under our contractual agreement, your insurance company has 30 days to pay claims. Expenses incurred are the responsibility of the patient, not the insurance company. Should your claim be denied, payment in full is due immediately. If your insurance company has not responded to our claim within 60 days from the date of your office visit, the entire balance will be transferred to you for immediate payment. Also, bear in mind that medical insurance covers only medically necessary treatments and services. All cosmetic and elective services are not covered by insurance and will be out of pocket expenses for you.
There will be a $30 fee for all returned checks.
